This information will be kept confidential and is intended to help the teacher/director understand the needs of your child and to help your child feel more comfortable upon entering this new environment.

Medical Permissions

I give permission for a staff member to administer emergency medication (EpiPen or inhaler) that I have provided for my child.

I give permission for the Director and teachers to have access to my child's health information in the school file.

I understand that, in some emergency situations, the staff will need to contact the Emergency Medical Service (911) before the parent, child's physician, or other adult acting on my child's behalf is contacted. If time permits, my child should be transported to the hospital listed below. If it is a life-threatening emergency, I understand my child will be transported, at my own expense or at the expense of my insurance carrier, to the appropriate hospital. If no hospital is designated, we will transport to St. Clair Hospital in Mt. Lebanon, PA.

I hereby grant the staff of Tender Shepherd Preschool to take whatever emergency measures are necessary for the care and protection of my child while under the care and supervision of the preschool.

Readiness Guidelines

In determining whether our program is an appropriate fit for your child, please indicate whether your child meets these behavioral guidelines. While it is Tender Shepherd Preschool's goal to assist in achieving these goals, we would like to have an idea of where your child is at the present time.

Tender Shepherd Preschool Contracts

I/We agree to the following:

1. My child may use the play equipment and participate fully in the directed activities of the teaching staff.

2. The preschool staff will be responsible for my child only during classroom sessions. The preschool will not be responsible for my child traveling to and from school.

3. I understand and will abide by the guidelines presented in the Tender Shepherd Preschool Handbook.

4. You have my permission to obtain emergency medical care if a family member cannot be reached. Any expense incurred in obtaining such care will be my responsibility.

5. If my child becomes ill at school and I cannot be reached, Tender Shepherd Preschool will contact the persons listed as emergency contacts.

6. I agree to pay Tender Shepherd Preschool for the 2024-2025 academic year. Tuition is based on an annual fee; therefore, no credit is allowed for illness, holidays, or vacations. The first month's tuition will be paid at the time of registration for new families or on May 1, 2024, for returning families and is non-refundable. A late fee of $25 will be assessed for payments received after the 10th of each month. All tuition must be paid online.

WAIVER, RELEASE, AND ASSUMPTION OF RISK FORM

On behalf of myself, my household members, and any minor child(ren), I hereby give permission for my child to attend the event noted in the event registration or verify that I am attending the event at The Bible Chapel (any campus). My child and I are familiar with and knowingly and voluntarily accept all risks associated with attending the event at the campus noted in the event registration. I acknowledge our participation in this program is wholly voluntary.

I specifically assume all risks and hazards associated with our participation. I understand that we will be associating with staff, volunteers, and other children and may contact viruses (including COVID-19) and diseases through our participation. 

While instruction and reasonable supervision will be provided, staff and volunteers cannot ensure our safety. Accidents and injuries happen, and it is impossible to eliminate the risk that we will suffer an injury or illness.

I certify that we are in good health, have no fever, and have no current issues that make it unsafe for us to participate in the event, which may or may not have a medical professional on-site. I acknowledge that my child and I are responsible for ensuring that he or she takes any necessary medication and for avoiding any allergies. In the event of a medical emergency, 911 will be called, and I will be responsible for all costs of medical treatment.

By signing this form, I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of others, and assume full responsibility for our participation at The Bible Chapel.

PHOTO RELEASE: I hereby agree on behalf of myself and my child to authorize The Bible Chapel to use any photographs, videos, and electronic recordings of me and or my child(ren) for Bible Chapel promotional purposes. I understand that my child's image may be shared through photos or videos via the Tender Shepherd Preschool Homeroom App, Social Media, or Classroom Journals.

MEDICATION DISTRIBUTION: I give my consent for the emergency medication prescribed to be self-administered by my child and/or assisted by an adult at the event as needed. I understand that volunteers will make every attempt to reach the parent if these medications are administered and that if an Epi Pen/Auvi-Q is administered, 911 will be called for EMS response.

Tender Shepherd Preschool: Health and Safety Plan

Tender Shepherd Preschool desires to ensure the health and well-being of each child in the school.  These guidelines have been carefully considered to address and promote the safety, health, and welfare of our community. 

Tender Shepherd Preschool/The Bible Chapel Health Guidelines

Please review our health guidelines before participating in any Bible Chapel ministry, program, or event.  For the safety of our volunteers and those participating in Bible Chapel ministries, programs, or events, anyone experiencing the following should not participate in any activity at The Bible Chapel:

Those who have:

  • an acute cold
  • constant cough or sneezing
  • earache or sore throat
  • red or running eyes
  • infectious mucous drainage
  • rash or skin eruptions
  • fever, vomiting or diarrhea
  • been diagnosed with covid-19
  • encountered anyone diagnosed with covid-19
  • taken medication that could mask symptoms
  • started taking antibiotics within the past 48 hours

 

Children should be free of these symptoms for at least 24 hours before returning to school.

Those who have had any symptoms of a communicable illness (covid-19 or otherwise) within the past 72 hours should not attend preschool.

Registration Information

The Preschool Director will determine placement upon review of the registration. Completion of the necessary registration form and fee does not guarantee placement or acceptance into our program.

Non-refundable fees:

~ A registration fee of $50 is required.

~ First month of tuition in the amount of $195 for three-day preschool or $230 for four-day preschool (new families, due at time of registration; returning families, billed May 1, 2024).
